Federal Form 3 “Notice to Employees” Required for Radiological Working Conditions

NRC Form 3, ‘Notice to Employees” (CPC Poster #74216) is the mandatory federal radiation notice that must be posted in facilities licensed or regulated by the federal Nuclear Regulatory Commission (NRC). In addition, state facilities and activities that possess a license from the applicable state regulatory agency may be required to post both the NRC Notice and the state regulatory agency’s notice.

Specifically, those facilities and activities that receive, possess, use, or transfer material licensed or regulated by the NRC must post the NRC’s current Form 3. (10 CFR 19.11) Radiological working conditions that generally involve NRC licensure include hospital, medical and veterinary facilities, research and academic institutions, transportation, storage and disposal of regulated materials, fuel and energy, construction, industrial and manufacturing applications.

The Notice provides current information regarding standards for protection against radiation; notices, instructions and reports to workers; inspections; and employee protection. The Notice also provides contact information for regional office locations of the NRC and hotline numbers to report safety concerns and incidents. Under the regulation, the Notice must be posted in a sufficient number of places to permit individuals engaged in NRC-licensed or regulated activities to observe them on the way to or from any particular licensed or regulated activity location to which the document applies, must be conspicuous, and must be replaced if defaced or altered.
