The Utah Civil Rights Notice was created by the Utah Department of Human Services to inform individuals that the licensed program complies with applicable Federal civil rights law and does not deny services on the basis of race, color, sex, religion, disability, age or national origin.
A contact person is provided on the poster for individuals that feel the program failed to provide services or discriminated in another way on the basis of race, color, sex, religion, disability, age or national origin.
Posting Requirement
All Utah human service licensed programs or providers must display this civil rights poster in a conspicuous place. License types include: adoption adult day dare, day treatment, foster care, intermediate secure care, outdoor youth program, outpatient treatment, recovery residence, residential treatment, social detox, and therapeutic schools.
