The Utah Provider Code of Conduct Poster was created by the Utah Department of Human Services to inform individuals that any contracted, licensed or certified agency, individual, or employee that is aware of, or suspects abuse, neglect, harm, mistreatment, fraud or exploitation is required to report it. A phone number is provided on the poster for any individual that witnesses a code of conduct violation and would like to make a complaint. The poster also includes a list of phone numbers for the following departments to report concerns or complaints regarding questionable financial activity or other unethical behavior:

  • Adult Protective Services
  • Department of Human Services Office of Licensing
  • Office of Child Protection Ombudsman
  • Child Protective Services
  • Bureau of Internal Review and Audit

Posting Requirement

All Utah human service licensed programs or providers must display this code of conduct poster in a conspicuous place. License types include: adoption, adult day care, day treatment, foster care, intermediate secure care, outdoor youth program, outpatient treatment, recovery residence, residential treatment, social detox, and therapeutic schools.
