Compliance Poster Company reminds all employers covered by the San Francisco Health Care Security Ordinance (HCSO) of the requirement to submit a 2011 Annual Reporting Form (ARF) by April 30, 2012. This requirement applies to covered employers. Covered employers are
- Private employers with 20 or more employees (including those employed outside of San Francisco) in each of the four calendar quarters of 2011; or
- Non-profit corporations employing 50 or more employees (including those employed outside of the City) in each of the four calendar quarters of 2011.
Those employers not covered by the HCSO should not submit the 2011 ARF. Amendments made to the Ordinance in November 2011 require an increased penalty of $500 per quarter for failing to submit the ARF. Click here for instructions guiding employers through each question.
To launch and submit the 2011 ARF as an online, fillable PDF form click here. Employers need to have Adobe 9.0 or higher to complete the ARF.
In addition to the reporting requirement, the November 2011 amendment requires covered employers to post the Official Office of Labor Standards Enforcement (OLSE) Notice in a conspicuous place at any workplace or job site where any covered employee works. Every covered employer must post the Official Notice in English, Spanish, and Chinese. The Official Notice must also be posted in any other language spoken by at least 5 percent of the employees at the workplace or job site. Covered employers are required to post the Official Notice at every workplace where even a single covered employee works. The OLSE may impose administrative penalties of $25 per day for each workplace or job site where a covered employer fails to post the Official Notice.
Compliance Poster Company has the HCSO Official Notice as well as two other mandatory to post notices (SF Minimum Wage and Paid Sick Leave) with six languages on one poster #05040.
